COVID RESPONSE VOLUNTEER SELF-DECLARATION

The safety of our employees, volunteers, families and visitors remain a priority for Habitat for Humanity of the Greater Teton Area (Teton Habitat).

To prevent the spread of COVID-19 and reduce the potential risk of exposure, we are asking volunteers to acknowledge this simple screening questionnaire.  Your participation is important to help us take precautionary measures to protect you and everyone you work with.

  1. I am not experiencing any cold or flu-like symptoms (to include fever above 100.4 degree, cough, sore throat, respiratory illness, difficulty breathing).
  2. I have not had any close contact with or cared for someone diagnosed with COVID 19 or someone who is experiencing any cold or flu-like symptoms in the last 14 days.
  3. I have not been in close contact with anyone who has traveled to any country outside of the United States within the last 14 days.

I understand that if any of the above statements are false, I am ineligible to volunteer for a minimum of 14 days.

I will inform Teton Habitat if, within 14 days following my volunteer shift at the Grove, I or develops any health problems related to “Coronavirus-COVID-19”.

I will strictly follow all safety precautions required by Teton Habitat and will abide by the judgement of Teton Habitat staff if I violate those precautions.
